APD Assessment Questionnaire

Thank you for choosing to have your child’s listening ability assessed at Newcastle Hearing Centre for Children. In order to get to know your child and their listening difficulties we request that you complete the following questionnaires prior to your appointment. If you have any additional information that you are happy to share, such as psychologist or speech pathologist reports, please forward them to us.
